The Admissions Process
Throughout the
admissions process, our Admissions Director, Nuria Grant, is
available to answer all of your questions and to let you know what
information is needed for our Admissions Committee. Nuria can be

Selecting a Facility
We encourage
families and friends to visit several skilled nursing facilities
before choosing the one that they feel would be best. We offer tours
of Ozanam Hall every Thursday at 11:00 A.M. If that time is not
convenient for you, our Admissions Department will be happy to
schedule a tour that meets your needs.

After a
Facility is Selected
If your family member is in the
hospital and may require short term rehabilitation or long term
skilled nursing care, contact our Admissions Director who will
expedite the discharge plans. In order for our Admissions
Committee to consider a resident for placement in Ozanam Hall, a
Patient Review Instrument (PRI) must be completed. The PRI provides
pertinent information regarding the medical/rehabilitation needs of
the patient. If your family member is at home, the PRI will have to
be completed by a Registered Nurse assessor or an agency that
performs this service.
Availability of beds can vary from
day to day based on the person's needs. Our Admission Staff will do
everything possible to facilitate admission. Ozanam Hall admits
residents without regard to race, color, national origin, or
disability and abides by all applicable local, state and federal
