Palliative Care
Palliative Care at Ozanam Hall Nursing Home as defined by the Carmelite Sisters for the Aged and Infirmed:
Palliative Care seeks to provide relief from the 5 domains of pain (Physical Pain, Emotional Pain, Psychiatric Pain, Spiritual Pain and Familial Pain) that all persons experience when faced with a debilitating diagnosis. The Carmelite Sisters have provided Palliative Care since 1929. In recent years they have implemented a formalized, professional, caring Palliative Care Program here at Ozanam Hall and in their other nursing facilities. Palliative Care is for anyone diagnosed with a serious or chronic condition and planning begins at the admission. All staff, the Resident and their Representative are part of our Palliative Care Planning Team and together- look to plan to provide treatment, comfort and the best possible care from the moment one is admitted and through-out their stay as needs are identified.
Though Palliative care is incorporated into Hospice and End of Life Care it is not reserved for the imminently dying.
We, at Ozanam Hall, create a Palliative Care Plan for every individual upon admission for rehabilitation or for long term care. We begin the process together ( with the Resident and their Representative) to aid in helping each Resident and their family communicate about their experience with the 5 domains of pain and start to plan how we can try to alleviate the pain and optimize their quality of life and regard Resident directed care. It is not about prognosis it is about living a life free from worry, emotional and physical/spiritual pain and give our Residents back control and comfort in their later days while decreasing the representatives fears or “feeling like walking on egg shells” around difficult discussions and choices.
Palliative Care is the beginning of an open, honest dialogue to help lessen and/or alleviate the stress one harbors about personal health needs/experiences, future treatment wishes, worries over relationships and role responsibilities and what our religious leader or our family expects from us or for us while making plans that respect and regard a Resident’s wishes..
If, over time, one’s situation becomes a serious and or imminent end stage of one’s life…we hope, by that time, we have made the Resident, their relatives and our staff ready to hand their loved one up to their final reward… without the weight of worry, pain or indecision.
Come, entrust your loved one’s care to us, where our staff champion our mission to help heal, provide symptom control, and educate all to the underlying disease process and plan to minimize pain in an atmosphere of love and caring to promote a life lived in comfort.
Our Long Term Skilled Nursing units specialize in geriatric care and are designed with private and semi-private rooms. Residents enjoy an outdoor garden - which is a perfect, tranquil place to relax or meditate - as well as three spa areas, a large day area, and a dining room featuring individualized meal service. On our main floor, we have a beautiful chapel where Mass is held daily. On Fridays we have a Rabbi who visits, and many other spiritual ministers visit regularly to offer spiritual care to our residents.
Our auditorium holds many different events and programs for residents, from bingo to musical functions. For more cognitively aware residents, we also offer a Main Floor dining room that has restaurant-like service.
Other amenities include:
